Wounds that are hard-to-heal and/or non-healing may be a result of pressure, trauma or infection. Sometimes, the simplest of wounds can turn into a significant problem due to underlying conditions such as diabetes, circulation problems or previous radiation treatment.
We are the only practice in Southern Cal to offer this service to Medicare and Medi-Cal Patients. An allograft amniotic membrane is a human tissue patch that's used to cover, protect, and nourish wounds. It's made from minimally manipulated amniotic tissue, which contains growth factors, cytokines, and anti-inflammatory properties that help wounds heal. Amniotic allografts can be used in a variety of surgical and wound treatment scenarios, including:
Spine, orthopedic, and neurosurgery: Amniotic allografts can be used to support surgical wounds. External wounds: Amniotic allografts can be used as a barrier for wounds like diabetic foot ulcers, venous leg ulcers, pressure ulcers, and burns. Most insurance plans are accepted including Medi-Cal. The National ASP has been released, and AHC-LA have a number of these current and scientifically validated Amniotic Membranes that could best suit your patient and practice needs. As a Wound Care Clinician at AHC-LA For the past 12 years AHC-LA has promoted the DNA Diagnostic Test for No-healing wounds. In collaboration with MicroGen RX and Southwest Regional Wound Center. Dr. Randy Wolcott always believed that amputation wasn't the solution to chronic wounds. And now he has found a way of healing without hurting his patients. It's a revolution in wound care and it's saving the limbs and lives of his patients.
A young woman with type I diabetes, who had lost her kidney transplant a year previously, was hospitalized twice over six months with sepsis — the source of which remained a mystery. A large transplant surgery abdominal wound had been open for months, as had a heel pressure sore complicated by peripheral arterial disease, but both improved with better nutrition and regular wound care. The patient then presented with sudden left leg pain, swelling, and the rapidly ascending erythema characteristic of necrotizing infection.
I run all the clinical research, wound healing program that we do here. Mainly do a lot of limb salvage, diabetic foot reconstruction, wound healing. Been doing it for 12 years. When somebody comes to you and says, "This is the greatest thing ever", this will do everything for your patient. You need to be skeptical about that. We're not saying that this is going to cure everybody but it's going to put you on that path to get to that point where we can fix a lot of these problems. It's science, science changes, science evolves. We as clinicians need to keep open minds about things, but we do need to see the evidence.
The initial thoughts where I thought it was neat that we are using DNA finally to do that. I was very skeptical how it was going to be. But I can tell you, one of the first cases I ever did, I had a gentleman with the venous leg ulceration, 77 square centimeters. Did the DNA testing, we'd made him the cream. And after two applications, he shrunk down to about seven centimeters. And just after two applications. He had bad COPD, bad emphysema, a lot of refluxes, a lot of lymphedemas in his feet. I have the pictures to prove all that. That was one of the most salient features I noticed about the DNA accurately predicting what's in that wound so we can treat that. We can remove the chronic inflammation which keeps these wounds open. So, that was quite dramatic.
"MicroGen and your company is the best that's happened in this space, and as long as I have been in my career. You need to know about this technology, and you need to know about it yesterday. For the majority of patients that I see with my complex urology practice, traditional antimicrobial culture testing and sensitivity has just failed to show me what I need. I was a little skeptical at first to see what this would bring. What I was surprised so quickly after starting a therapy was not how many negatives there were, but how many positives they were. In fact, how few negatives they were. "To say that I have a traditional culture negative and I find two to five organisms on a MicroGen that I have to treat before I instrument or tell a patient that they're infection free. To me, that huge gap in information is something that I cannot fathom treating my patients without in the future.
"I never used to take the culture results into the patient. Now, it's an exciting part of the patient teaching experience. You can show the three phases of it, the PCR phase, genetic resistance patterns, then the final evaluation with the usually multiple organisms and multiple sensitivity, it's fun to say, "Okay, what's that one antibiotic or the fewest number of antibiotic I can come up with to get you on your course and to get you treated?" Let's decide how long based on your pattern and intensity of your disease.

Path Toward Healing
Wound care specialists at Alternative Healthcare LA help stimulate the body's restorative properties by removing dead tissue and then sterilizing and dressing wounds. "Most patients we treat are people with diabetes who have lower-extremity wounds, such as pressure ulcers in their feet or ankles, at AHC-LA Treatment begins with the painless removal of the dead tissue: callous skin is excised manually or dissolved with enzymatic treatments. Depending on the presence of infection, topical or intravenous antibiotics are delivered to the wound site. Some wounds can be closed with Amniotic skin grafts.
Hyperbaric Oxygen Therapy
A patient with a lack of oxygen or blood flow to a limb, a condition known as ischemia, may be a candidate for hyperbaric oxygen therapy (HBOT). This type of treatment delivers pressurized oxygen to the wound site to help speed the healing process. "During HBOT, oxygen is delivered at two-and-a-half to three times atmospheric pressure, stimulating new blood vessel and cell growth to promote quick healing. A typical course of HBOT lasts up to eight weeks of treatment. During therapy, patients rest comfortably in a hyperbaric chamber where they may relax, sleep or watch television.
Case managers assist patients and their family members with arrangements for transportation, home healthcare, durable medical equipment and other services. Some patients may be outfitted with off-loading devices, such as surgical boots and casts that relieve wound-site pressure to speed healing.
